Welcome to the Connecticut Challenge survey of cancer survivors.
We need your help for 10 minutes.

The Connecticut Challenge was founded in 2005 to provide support services to help cancer survivors prevent or overcome potential lingering effects of the disease and the harsh treatments you underwent to defeat it. We fund the only cancer survivorship clinics in Connecticut, at Yale Cancer Center: the Connecticut Challenge Adult Survivorship Clinic and the HERO'S Clinic for survivors of childhood cancers.

We are working to add new resources and to make them available to the widest possible audience of survivors. Whether you live in Connecticut or not, by completing this anonymous survey, you are helping us learn more about the issues cancer survivors face, the support services you need and want and the most effective ways to deliver them to you.

Please take this survey if:

  • You were diagnosed and actually treated for cancer
  • You have completed active cancer treatments (surgery, chemotherapy, radiation)

DO NOT take this survey if:

  • You are currently awaiting or undergoing surgery, chemotherapy, radiation or some other active cancer treatment
  • You had a close call with cancer, but never received a positive diagnosis
